Virtual Case Manager

Virtual Case Manager is a comprehensive shared database that has tracking, referral, resource and report writing capabilities. Virtual Case Manager, or VCM, shares information between agencies and organizations allowing for greater collaboration, communication and production of services. For example, if a resident of Heartside needs two prescriptions filled, one being a psychotropic medication, they might head to Degage to get these prescriptions filled. Unknown to the resident, Degage is unable to fund psychotropic medications. Using VCM, Degage can refer the individual to Heartside Ministry who can fund psychotropic medications. The use of VCM will lead to greater communication and collaboration in the community which will, in turn, reduce unnecessary duplication of services and provide more comprehensive and coordinated services to those in need.
